In women, estrogen is primarily produced in the ovaries, but fat cells and adrenal glands contribute too, especially as women age or experience menopause. For men, the adrenal glands and fat cells are also sources of estrogen, but in smaller amounts. Though often associated with women’s health, estrogen plays critical roles in many areas of health for both genders.

1. The Menopause Transition


  • Menopause occurs after 12 months with no menstrual cycle, marking the end of a woman’s reproductive years. With life expectancy now reaching well beyond reproductive age, most women spend about 40% of their lives post-menopause, making estrogen loss an essential focus.

  • When estrogen declines, common symptoms like hot flashes, night sweats, and mood changes emerge. Estrogen levels can start to drop during perimenopause, leading to erratic menstrual cycles and symptoms similar to those experienced in puberty.


2. Estrogen and Heart Health


  • Estrogen is a protective agent for the heart, reducing cholesterol plaque formation in arteries and thus lowering the risk of heart attacks and strokes. Studies show that hormone replacement therapy (HRT) for menopausal women can significantly reduce heart disease risks.

  • In a study of 41,000 women, those on hormone therapy had lower cardiovascular death rates compared to those who were not, proving estrogen’s vital role in maintaining heart health.


3. Bone Health and Osteoporosis Prevention


  • Estrogen is essential for maintaining bone density. During menopause, estrogen levels plummet, accelerating bone loss and increasing the risk of osteoporosis. The loss can be rapid, with women experiencing up to a 12% bone density loss within the first few years of menopause.

  • Maintaining high bone density through exercise and estrogen therapy in younger years can help protect against osteoporosis later in life.


4. Brain Health and Cognitive Protection


  • Estrogen has neuroprotective effects and helps prevent the accumulation of beta-amyloid plaques, which contribute to dementia and Alzheimer’s. Studies have shown that estrogen therapy may prevent or even reduce these plaques, especially when started before cognitive decline begins.


5. Genitourinary Health


  • Loss of estrogen can cause genital and urinary issues, collectively known as the Genitourinary Syndrome of Menopause (GSM). Symptoms include dryness, pain during intercourse, and recurrent urinary tract infections (UTIs). Estrogen therapy can often relieve these symptoms, providing significant quality-of-life improvements.

While estrogen is less discussed in men’s health, it’s no less important. Men produce estrogen primarily by converting testosterone into estrogen through a process called aromatization.


1. Heart Health


  • Just like in women, estrogen protects men’s cardiovascular systems. Studies have shown that men with both low and excessively high estrogen levels are at an increased risk for heart disease. Maintaining balanced estrogen levels is crucial for heart health, especially as men age.


2. Bone Health


  • Estrogen plays a role in maintaining bone density in men as well. As testosterone levels naturally decline with age, estrogen levels may drop too. This can lead to increased bone loss and a greater risk of osteoporosis, underscoring the need for balanced estrogen levels in aging men.


3. Brain Health and Libido


  • Estrogen’s neuroprotective benefits extend to men, helping to reduce the risk of cognitive decline and dementia. Low estrogen levels in men can also correlate with lower libido. So when men undergo testosterone therapy, some increase in estrogen is beneficial for preserving libido and protecting against cognitive decline.
